Introduction: Empathy plays an essential role in improving communication between people and has a positive and constructive nature and causes cooperation and unity within the organization. The purpose of this research was to investigate the effective factors in organizational empathy and its role in organizational citizenship behavior in Tehran military hospitals. Method: The present study is survey research that was conducted in 2023 in Tehran. The statistical population included all employees and personnel of the hospitals in Tehran, and the sample size estimated 335 which was calculated based on Cochran’s formula. The subjects were selected by stratified random method. Descriptive statistics, t-test and Cronbach’s alpha coefficient were used to analyze the findings. Finding:The resultsof the research showed that organizational factors have a positive and significant effect on organizational citizenship behavior. But environmental factors have no significant effect on organizational citizenship behavior. Also, managerial factors have a positive and significant effect on organizational citizenship behavior, and finally, the results of the fifth hypothesis test showed that ethical factors do not have a significant effect on organizational citizenship behavior. Discussion and conclusions: Occurrence of unusual anti-citizen behaviors such as lack of work, aggression, bullying, stubbornness, and intimidation affect the performance of organizations and interpersonal relationships and the spirit of employee cooperation.
