fa تحلیل ریشه ای یک رویداد ناگوار آسیب به بیمار به علت افتادن از تخت متعاقب سنکوپ در یکی از بیمارستان های ایران: گزارش موردی The bed following syncope in a hospital in Iran: A Case Report تخصصي Special مروری Review مقدمه: انسان در معرض خطا و اشتباه قرار دارد اما ارائه مراقبت های بهداشتی و درمانی مسئولیت بسیار سنگینی است و اندازه گیری رویدادهای پزشکی جزء اصلی ایمنی بیمار محسوب می شود. تحلیل ریشه ای وقایع یکی از پرکاربردترین ابزارهایی است که محیط را دریافت ایده های افراد تسهیل می نماید و فراتر از درگیر کردن فرد یا اشخاص است. هدف مطالعه ی حاضر تحلیل ریشه ای یک حادثه در یکی از بیمارستان های ایران می باشد. روش کار: این مطالعه یک پژوهش کیفی است و مهم ترین مراحل آن عبارتند از: شرح حادثه، تشکیل تیم، جمع آوری داده ها با استفاده از روش های مختلف(بارش افکار، مصاحبه، مرور اسناد، تحلیل تغییر، گروه اسمی و نمودار استخوان ماهی) تشخیص علل و عوامل ریشه ای و نهایتا ارائه و پیشنهاد راه حل ها. یافته ها: هفت عامل اصلی این رویداد فوق عبارتند از: 1. شلوغ و به هم ریخته بودن اتاق درمان، 2. گرفتن مسیر وریدی از بیمار در وضعیت نشسته ،3. پذیرش بیمار اضافه بر تختهای مصوب، 4. فضای محدود اتاق درمان جهت انجام روندها، 5. سپری شدن حداکثر وقت پرستار در مستند سازی و تکمیل اوراق پرونده، 6. تنها گذاشتن  بیمار در اطاق درمان توسط پرستار و 7. تعجیل در آماده نمودن بیمار جهت انتقال به اطاق عمل. آنگاه پیشنهاداتی برای حذف هریک از عوامل ریشه ای ارائه گردید. نتیجه گیری: تغییر رویکردهای سیستمی، تغییرات سازمانی و استقرار استانداردها در هر سیستم از مهمترین مسائلی پیشگیری از حوادث آینده قلمداد می گردند. تمام مدیران و مسئولان بیمارستان ها رویکرد تحلیل ریشه ای وقایع را در دستور کار مدیریت خطر در سازمان خود قرار دهند. چرا که هر سیستم ممکن است برخی مشکلات زیربنایی داشته باشد که در سیستم های دیگر وجود نداشته باشد. Introduction: To err is human but the delivery of healthcare is an enormous responsibility and detection and measurement of medical events is the main component of patient safety. Root cause analysis (RCA) is one of the very useful tools that facilitates the environment in order to get people ideas and is beyond involving a person or individuals. The present study aims the root cause analysis of an adverse event in one of the hospitals in Iran. Methods: This study is a qualitative survey and the main steps of it are as follows: defining event, team formation, data collection using deferent methods (brainstorming, interview, documents reviewing, analysis of changes, nominal group and fish bone diagram), identifying the root factors and causes and ultimately providing and offering the solutions. Findings: The seven root causes of this event were as: cluttered treatment room, taking the patient’s IV line in sitting position, patient reception in addition to the beds approved, the limited space of the treatment room, the maximum elapsed time of nurse in documentation, placing the patient alone in the treatment room and rapid preparation of the patient. Then, suggestions for removal of any of the root causes were presented. Conclusion: Change of system approaches, organizational changes and the establishment of standards in each system is one of the most important issues in prevention of the future events.
