fa مطالعه هزینه- فایده خدمات مراقبت در منزل بیماران ترخیص یافته از یک بیمارستان نظامی شهر تهران در سال ۱۴۰۲ Cost-Benefit Analysis of Home Care Services for Patients Discharged from a Military Hospital in Tehran in 2023 تخصصي Special پژوهشي Research مقدمه: بستری شدن مجدد بیماران، به ویژه سالمندان و بیماران دارای بیماری‌های مزمن، رایج و پرهزینه است. مراقبت در منزل به عنوان یک روش مراقبتی مبتنی بر جامعه، مزایای فراوانی برای بیماران و نظام سلامت دارد. هدف پژوهش حاضر ارزیابی هزینه-فایده خدمات مراقبت در منزل برای بیماران ترخیص یافته از بیمارستان نظامی تهران در سال ۱۴۰۲ می باشد.  روش کار: این پژوهش توصیفی-کاربردی بوده و از نظر جمع آوری داده ها به روش بررسی پرونده های بیماران ترخیص شده از بیمارستان انجام شده است. جامعه آماری شامل 915 بیمارترخیص شده با محدودیت حرکتی از بیمارستان نظامی منتخب شهر تهران در سال1402 می باشد که بعد از محاسبه معیارهای ورود و خروج پژوهش تعداد839 بیمار جهت انجام پژوهش انتخاب شدند. برای تحلیل داده‌ها از آمار توصیفی و استنباطی، از جمله رگرسیون لجستیک، و نرم‌افزارهای Eviews ورژن11 و  SPSSورژن 26 استفاده شده است. این مطالعه با کد اخلاق 1403.155IR.IAU.TMU.REC در دانشگاه علوم پزشکی آزاد اسلامی تهران تصویب شد. یافته ها: نتایج نشان داد که ارائه خدمات بهبود یافته درمان در منزل، از جمله تسهیل در تهیه تجهیزات دارویی از طریق نسخه الکترونیک و حضور نیروی کادر درمان در منزل، باعث افزایش کیفیت خدمات به ترتیب ۱۲، ۱۵ و ۱۶ درصد شده است. این افزایش کیفیت موجب رضایتمندی بیشتر بیماران و افزایش درآمد کلی بیمارستان گردیده است. نتیجه گیری: ارائه خدمات مراقبت در منزل پس از ترخیص منجر به بهبود کیفیت خدمت، کاهش هزینه‌ها و تحقق اهداف سلامت شده و در زمینه‌های فرهنگی و اجتماعی نیز به نفع بیمار، جامعه و دولت است.   Introduction: Readmission of patients, especially the elderly and patients with chronic diseases, is common and costly. Home care, as a community-based care method, has many benefits for patients and the health system. The aim of this study is to evaluate the cost-benefit of home care services for patients discharged from Tehran Military Hospital in 2023. Methods: This is a descriptive-applied study and in terms of data collection, it was conducted by examining the records of patients discharged from the hospital. The statistical population includes 915 patients discharged with limited mobility from a selected military hospital in Tehran in 1402, and after calculating the inclusion and exclusion criteria, 839 patients were selected for the study. Descriptive and inferential statistics, including logistic regression, and Eviews version 11 and SPSS version 26 softwares were used to analyze the data. This study was approved by the Ethics Committee of Islamic Azad University Tehran Medical Sciences (Ethics code: IR.IAU.TMU.REC.1403.155). Results: The results showed that providing improved home care services, including facilitating the provision of pharmaceutical supplies through electronic prescriptions and the presence of medical staff in the home, increased the quality of services by 12, 15, and 16 percent, respectively. This increase in quality led to greater patient satisfaction and increased overall hospital revenue. Conclusion: Providing home care services after discharge leads to improved service quality, reduced costs, and the achievement of health goals.
