fa تحلیل مفهوم مراقبت تسکینی زخم: رویکرد هشت‌مرحله‌ای واکر و آوانت تخصصي Special مروری Review مقدمه:<br> مراقبت تسکینی زخم یک رویکرد کل‌نگر و انسان‌محور برای مدیریت زخم‌های پیچیده و غیرقابل‌التیام در بیماران مبتلا به بیماری‌های پیشرونده است. با وجود اهمیت رو‌به‌افزایش آن، ابهام مفهومی و نبود یک تعریف عملیاتی منسجم، موجب محدودیت در پژوهش و چالش در تفکیک آن از مراقبت‌های معمول زخم شده است. ازاین‌رو، تحلیل و شفاف‌سازی این مفهوم برای ارتقای کیفیت مراقبت ضروری است.<br> مواد و روش‌ها:<br> این مطالعه یک تحلیل مفهوم کیفی بر اساس روش هشت‌مرحله‌ای واکر و آوانت بود. داده‌ها از طریق مرور نظام‌مند متون علمی از&nbsp;<span dir="RTL" lang="AR-SA" style="font-size:11.0pt"><span style="background:yellow"><span style="line-height:115%"><span b="" nazanin="" style="font-family:">سال 2005 </span></span></span></span>تا ۲۰۲۵ گردآوری شد. پس از غربالگری، ۱9 منبع واجد شرایط وارد تحلیل شدند. مراحل شامل شناسایی کاربردهای مفهوم، تعیین ویژگی‌های تعریف‌کننده، ساخت موارد نمونه و مرزی، شناسایی پیش‌نیازها و پیامدها و در نهایت ارائه تعریف عملیاتی بود.<br> یافته‌ها:<br> تحلیل نشان داد که مراقبت تسکینی زخم دارای پنج ویژگی اصلی است: تغییر هدف از ترمیم زخم به ارتقای کیفیت زندگی، مدیریت فعال علائم آزاردهنده، نگاه جامع به ابعاد جسمی، روانی، اجتماعی و معنوی بیمار، مشارکت بیمار و خانواده در تصمیم‌گیری‌ها و رویکرد بین‌رشته‌ای. تدوین موارد نمونه و مرزی به روشن‌شدن حدود این مفهوم کمک کرد. پیش‌نیازهای اجرای موفق شامل وجود زخم با پیش‌آگهی ضعیف، بار بالای علائم و تصمیم آگاهانه برای تغییر رویکرد بودند. پیامدها نیز شامل کاهش رنج، بهبود کیفیت زندگی و کاهش بار مراقبتی بودند.<br> نتیجه‌گیری:<br> مراقبت تسکینی زخم یک رویکرد متمایز و مبتنی بر انسان‌محوری است که تمرکز آن بر التیام فرد به‌جای التیام بافت است. تعریف عملیاتی ارایه شده می تواند مبنایی برای استانداردسازی آموزش باشد. Introduction:<br> Palliative wound care is a holistic, person-centered approach for managing complex, non-healing wounds in individuals with progressive illnesses. Despite its growing relevance, the absence of a unified operational definition and conceptual ambiguity has limited its application in clinical practice and research. Therefore, clarifying this concept is essential to differentiate it from conventional wound care and improve care quality.<br> <br> Methods:<br> This study utilized Walker and Avant&rsquo;s eight-step concept analysis method. <span style="font-size:12.0pt"><span style="line-height:115%"><span new="" roman="" style="font-family:" times=""><span style="color:#0f1115">Data were collected through a systematic review of literature in international scientific databases from <span style="background:yellow">2005 to 2025</span></span></span></span></span>. After screening, 19 eligible sources were included. Analytical steps involved identifying uses of the concept, determining defining attributes, developing model and borderline cases, identifying antecedents and consequences, and formulating an operational definition.<br> <br> Results:<br> Five defining attributes of palliative wound care were identified: shifting the goal from wound healing to maximizing quality of life; active management of distressing symptoms; a holistic perspective addressing physical, psychological, social, and spiritual dimensions; collaborative care planning with patients and families; and an interdisciplinary approach. Model and borderline cases clarified conceptual boundaries. Antecedents included the presence of a wound with poor healing prognosis, significant symptom burden, and an informed decision to shift the care paradigm. Consequences included reduced suffering, improved quality of life, and decreased caregiver burden.<br> <br> Conclusion:<br> Palliative wound care represents a distinct, systematic philosophy centered on person-focused rather than tissue-focused healing.
