fa تاثیر درمان زود هنگام ضد قارچ در بیماران بستری در بخش مراقبت های ویژه با کاندیدا اسکور بالا Impact of Early Antifungal Therapy in ICU Patients with High Candida Scores تخصصي Special پژوهشي Research اهداف: عفونت‌های قارچی مهاجم، به‌ویژه عفونت‌های ناشی از گونه‌های کاندیدا، از علل اصلی مرگ‌ومیر در بیماران بستری در بخش مراقبت‌های ویژه (ICU) هستند. تشخیص و درمان دیرهنگام این عفونت‌ها می‌تواند به وخامت بالینی منجر شود. نمره کاندیدا به‌عنوان یک ابزار پیش‌بینی‌کننده برای شناسایی بیماران پرخطر به‌کار می‌رود تا درمان ضدقارچی زودهنگام آغاز شود. این مطالعه با هدف بررسی روند بهبود عملکرد اندام‌ها با استفاده از نمره SOFA در بیماران مبتلا به سپسیس و نمره کاندیدای بالای 3 که تحت درمان سریع ضدقارچی قرار گرفتند، انجام شد. روش‌ها: این مطالعه توصیفی–تحلیلی به‌صورت مقطعی در سال 2016 در بیمارستان امام رضا انجام شد. 30 بیمار ICU با سپسیس و نمره کاندیدای بالای 3 وارد مطالعه شدند. بیماران مبتلا به نوتروپنی یا بدخیمی‌های خونی از مطالعه حذف شدند. داده‌های دموگرافیک، علت بستری و نمرات SOFA در روزهای اول، سوم و هفتم ثبت و با آزمون تی زوجیp<0.05) سطح معناداری) تحلیل <a name="_Hlk205410159">شدند.</a> یافته‌ها: میانگین سنی بیماران 62 سال بود و 53.3٪ مرد بودند. شایع‌ترین دلایل بستری، پنومونی (33.3٪) و سکته مغزی (30٪) بودند. نمرات SOFA از روز اول (12.23) به روز سوم (10.70) و سپس به روز هفتم (9.06) کاهش معنی‌داری داشتند (p<0.0001). نتیجه‌گیری: درمان ضدقارچی زودهنگام در بیماران با نمره بالای کاندیدا باعث بهبود عملکرد اندام‌ها شده و از پیشرفت نارسایی ارگان‌ها جلوگیری می‌کند. استفاده از نمره کاندیدا برای شروع مداخله سریع توصیه می‌شود.   Introduction: Invasive fungal infections, particularly those caused by Candida species, are significant contributors to morbidity and mortality in critically ill patients, especially those admitted to Intensive Care Units (ICUs). Delays in treatment lead to poor clinical outcomes. The Candida score is a predictive tool developed to identify high-risk patients. This study aimed to evaluate the clinical prognosis of ICU patients with sepsis and a Candida score greater than 3 who received early antifungal treatment. Specifically, it focused on assessing changes in Sequential Organ Failure Assessment (SOFA) scores on days 1, 3, and 7 of therapy to determine the impact of early intervention on organ function. Methods: A cross-sectional study was performed at Imam Reza Hospital in 2016. Thirty ICU patients with sepsis and a Candida score above 3 were enrolled. Exclusion criteria included hematologic malignancies and neutropenia. Demographic data, reasons for ICU admission, and SOFA scores were collected from patient files. Statistical analysis was performed using paired t-tests with significance set at p<0.05. Results: The mean age of the patients was 62.06±18.20 years. Pneumonia was the most common reason for ICU admission (33.3%), followed by cerebrovascular accident (30%). The SOFA scores showed a statistically significant decrease from Day 1 (mean 12.23±2.62) to Day 3 (10.70±2.08), and further to Day 7 (9.06±2.05), showing marked clinical improvement. All pairwise comparisons between days were statistically significant (p<0.0001).
