A Case Report of Chronic Scrotal Infection Caused by Nontuberculous Mycobacteria in an Immunocompetent Individual After Prolonged Exposure to Pressurized Water

Introduction: Nontuberculous mycobacteria (NTM) are environmental
acid-fast bacilli commonly found in water and soil. They can
cause pulmonary infections as well as skin and soft tissue infections,
particularly after cutaneous inoculation due to trauma or exposure to
contaminated water. These infections often present with nonspecific
manifestations, and diagnosis may be delayed.
Case report: Here, we report a 38-year-old healthy, immunocompetent
man with chronic purulent scrotal discharge and no response to repeated
courses of antibiotic therapy. Despite extensive evaluation and
negative routine tests, molecular analysis of the scrotal discharge by
PCR was positive for nontuberculous mycobacterial infection.
Discussion and conclusion: This report highlights the importance of
maintaining clinical suspicion for NTM infection in patients with
chronic, treatment-resistant skin lesions, especially after exposure to
pressurized water, and underscores the value of molecular methods for
timely diagnosis |
