fa انواع جراحات و زخم های نظامی: مرور روایتی بر مکانیسم ها، پاتوفیزیولوژی، طبقه بندی و مدیریت مبتنی بر شواهد Military Wounds and War Injuries: A Narrative Review of Mechanisms, Pathophysiology, Classification and Evidence-Based Management تخصصي Special پژوهشي Research مقدمه: زخم های نظامی و رزمی از نظر مکانیسم انتقال انرژی، وسعت تخریب بافتی، بار آلود گی اولیه و بستر لجستیک د 0254 رمان، هویتی متمایز از ترومای غیرنظامی د ارند. د و د هه تجربه عملیات های عراق و افغانستان و سپس د اد ه های به د ست آمد ه از جنگ روسیه–اوکراین، هم الگوی آسیب و هم اکولوژی میکروبی این زخم ها را بازتعریف کر ده است. روش کار: این مطالعه یک مرور روایتی ساختاریافته است. جست وجو د 0254 ر پایگاه های ،Library Cochrane ،Science of Web ،Scopus ،MEDLINE/PubMed و نیز در 0254 مجموعه رهنمود های بالینی ،Magiran و Scholar Google ،Embase کمیته مراقبت تاکتیکی تلفات ،(JTS – System Trauma Joint) سامانه مشترک تروما Emergency و کتاب (ICRC) کمیته بی نالمللی صلیب سرخ ،(CoTCCC) رزمی انجام شد. بازه زمانی جست وجو 1990 تا ژوئیه 2026 بود . یافته ها: زخم های نظامی د 0254 ر هفت گروه مکانیسمی اصلی (نافذ/بالستیک، ترکشی، انفجاری، بلانت، له شد گی، سوختگی و ترکیبی) قابل دسته بند ی اند که د 0254 ر عمل اغلب هم زمان رخ می د هند . نتیجه گیری: بقای مجروح جنگی امروز بیش از آنکه محدود به کنترل خون ریزی باشد، به کیفیت مد یریت زخم، پیشگیری از عفونت مقاوم و بازتوانی جسمی–روانی وابسته است.   Introduction: Combat-related wounds differ from civilian trauma in energy transfer, extent of tissue destruction, initial contamination burden, and the logistic environment of care. Two decades of experience from Iraq and Afghanistan, and more recently data from the Russo-Ukrainian war, have redefined both injury patterns and the microbial ecology of these wounds. This review integrates current surgical, orthopedic and infectious-disease evidence on military wounds. Methods: A structured narrative review. PubMed/MEDLINE, Scopus, Web of Science, Cochrane, Embase, Google Scholar and Magiran were searched (1990 – July 2026), together with Joint Trauma System Clinical Practice Guidelines, CoTCCC TCCC Guidelines, ICRC war-surgery references and Emergency War Surgery. Results: Military wounds fall into seven mechanistic categories that frequently coexist. Their pathophysiology is governed by kinetic energy transfer, temporary and permanent cavitation, ischemia-reperfusion injury, systemic inflammation, and acute traumatic coagulopathy. Wound infection follows a time-dependent ecology: skin flora and clostridia predominate at the point of injury, whereas multidrug-resistant organisms are largely acquired along the evacuation chain. .
